Social Worker – Nursing home step-down service
Cpl Healthcare, in partnership with our client, a leading nursing home group, are recruiting for a Social Worker a new Nursing home opening in Dublin city.
This new nursing home is expected to have a 140-bed capacity and will provide a step-down nursing service for admissions with mixed clinical acuity and complex care needs along with the provision of long-term elderly nursing care.
This role would suit candidates with experience working within a primary care, rehabilitation or acute care setting working with the elderly.
The social worker will assist residents and their families with psychological, emotional, social or practical needs and advocate on their behalf as well assisting with various supports and entitlements including the Fair Deal Scheme and other support packages including home supports post discharge for some residents.
The duties of this role include:
· Working as part of a multi-disciplinary team, you will provide a comprehensive social work service to patients and families, to include psycho-social assessment, intervention, discharge planning and follow up or referral on, as required, to statutory and / or voluntary services.
· Empower clients / service users to advocate for their own needs, or where appropriate advocate on behalf of service users.
· Develop effective working relationships with and between clients / service users, statutory and non-statutory agencies and professionals.
· Attend multidisciplinary team meetings and case conferences as required.
Interested applicants should have:
•Professional Registration with CORU
•At least 1 year post-qualification experience
•Relevant experience working in a hospital, primary care or similar setting
•Experience working as part of a multi-disciplinary team
•Experience and knowledge of the Fair Deal scheme is desirable
